New Patients

We are happy to register new NHS patients within our catchment area.

How to register

To register with our practice, please print and complete the registration forms below. Every patient needs to complete the GMS1 form and then complete an additional form for each adult or child registering at the practice.

Online registration:

Register with a GP Surgery (gp-registration.nhs.uk)

No need to print and registration form is received through the Practice email.

After you have completed the form you must provide us with 2 forms of identification (ID) and your NHS no. (if you have registered with a GP before). Acceptable Identification can be tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.

Please fully complete, sign and scan with ID (for each person registering) before submitting via email to admin.moore@nhs.net or post all completed forms with ID through the letterbox at the practice.

Until you have submitted your ID you will not be registered at the practice. All incomplete registration forms maybe returned for completion.

Registration Medical

Once registered, make an appointment with our nursing team for a registration medical.

We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.
